The Role of Low-Molecular Weight Heparins in Non-ST Elevation Myocardial Infarction
نویسنده
چکیده
Non-ST elevation myocardial infarction (NSTEMI) is a subset clinical manifestation of acute coronary syndrome (ACS), which is usually caused by disruption of vulnerable atherosclerotic plaque, followed by thrombosis resulting various degree of occlusions in coronary arteries. The exposure of tissue factors following the plaque rupture causes activation of coagulation cascades and formation of factor Xa. The thrombin formation will cause fibrin deposition, platelet activation and finally lead to the formation of a stable plaque. Given the central role of thrombin in the pathogenesis of ACS, an antithrombotic agent is an important element in therapy ACS. Low-molecular-weight heparin (LMWHs) is one of antithrombotic agents which has been widely studied in various clinical trials and has been proven useful in clinical efficacy, safety and practical characteristics for ACS treatment. They have been found to improve clinical outcomes in acute coronary syndromes and to provide a more predictable therapeutic response, longer and more stable anticoagulation, and a lower incidence of UFH-induced

Low-molecular-weight heparins for the treatment of acute coronary syndromes.
Low-molecular-weight heparins (LMWHs) possess several advantages over unfractionated heparin (UFH) for the treatment of acute coronary syndromes (ACSs). Already a class I indication for the treatment of unstable angina/non-ST elevation myocardial infarction (UA/NSTEMI), LMWHs also show promise in the setting of ST elevation myocardial infarction (STEMI) and percutaneous coronary intervention (P...
متن کاملVitamin D deficiency predicts the ST elevation type of myocardial infarction in patients with acute coronary syndrome.
According to studies, a significant association exists between the low levels of vitamin D and cardiovascular diseases such as myocardial infarction (MI). In a prospective, case control study, 88 patients with acute coronary syndrome (ACS) including ST elevation myocardial infarction (STEMI) and Non-STEMI were enrolled. The plasma level of 25-hydroxy vitamin D[25(OH)D] was obtained at the time ...

متن کاملNew anticoagulant strategies in ST elevation myocardial infarction: Trials and clinical implications
New data have re-established the importance of anticoagulation of patients with ST segment elevation myocardial infarction (STEMI), both as an adjuvant to reperfusion therapy or in patients ineligible for reperfusion. Recent randomized trials have found newer agents to be superior to conventional unfractionated heparin. This article summarizes current understanding of the underlying pathophysio...

Since the Spanish Society of Cardiology Clinical Practice Guidelines on Unstable Angina/Non-Q-Wave Myocardial Infarction were released in 1999, the conclusions of several studies that have been published make it advisable to update current clinical recommendations. The main findings are related to the developing role of Chest Pain Units in the management and early risk stratification of acute c...
